COMMUNITY LISTENING TOUR

What are you seeing in your community?

Close the Gap is meeting with local physicians, practice managers, and community health professionals to better understand recurring barriers people face when navigating care and community resources.

We are asking for a brief 20–30 minute conversation centered on your practice-level observations and community experience.

WHY WE'RE LISTENING

Listen first. Build second.

We want future Close the Gap programs to respond to real community needs and complement the work local practices already do.

01

Understand recurring barriers

Where do people most often struggle with access, follow-through, cost, transportation, language, or understanding next steps?

02

Identify resource gaps

Which needs are hardest to address with the resources currently available in the community?

03

Learn what already works

Which services, referral pathways, or community supports should be strengthened rather than duplicated?

WHAT WE MAY ASK

A practical conversation, not a survey.

What needs come up repeatedly in your practice?

Where do people seem to get lost after a referral or recommendation?

Which resources are hardest to find, afford, understand, or access?

What do you wish more community members knew before a problem became urgent?

Where could education or resource navigation make the biggest difference?

WHAT THIS CONVERSATION IS

Low lift. Community focused.

This is a short discussion about patterns and community-level observations. We are not asking for funding, a formal commitment, or information that identifies an individual.

The goal is to help Close the Gap prioritize practical health education, resource navigation, and future community partnerships based on what local practices are actually seeing.

Could we learn from your practice?

If you are a physician, practice manager, nurse, care coordinator, social worker, or community health professional, we would value your perspective.
